Do you provide medical assistance? 

No. Assisted Transitions is a non-medical senior care service. Non-medical care includes companionship and supervision while preparing to transition either to homecare or an assisted living facility.

What is Move Management?

Senior Move Management is the process that assists older adults, individuals, and families with downsizing, relocation or simplifying their homes.

These services include: 

  • Developing an overall move or “age in place” plan

  • Organizing, sorting and downsizing

  • Interviewing, scheduling and overseeing movers

  • Arranging shipments and storage

  • Supervise and oversight of professional packing

  • Unpacking and setting up the new home


How much does it cost to work with Assisted Transitions?

Our Senior Living Advisement service is completely FREE. Additional services can vary considerably by services requested, and geographic location. After the initial consultation, we will outline the cost of services with a written estimate before proceeding. With that said, we strive to provide clear and cost-effective services to ease the stress of the move process for older adults and family members, not add to your concerns.
